The Fertility Institute of Hawaii (FIH) is transforming the dreams of parenthood into reality. As a leading fertility care clinic, it is committed to innovation and dedicated to helping patients create families through tailored fertility treatments and personalized care. In 2015, this resolve reached a significant milestone, marked by the record birth of 'one baby per day' in Hawaii, showcasing the profound success of its fertility interventions.
“Our ultimate goal is to help the community build the families they desire and provide them with the care they deserve and need,” says Dr. John Frattarelli, M.D., HCLD – medical, practice, and laboratory director.
Renowned for its state-of-the-art IVF lab and experienced staff, FIH offers technologically advanced and medically safe treatments that include ovulation induction, in vitro fertilization (IVF), intracytoplasmic sperm injection (ICSI), donor egg IVF, and fertility preservation.

Understanding individual patient’s needs and diagnoses is the primary step. Whether it involves insemination, donor sperm, donor eggs, donor embryos, IVF, genetic testing, or surrogacy, the clinic recognizes that every patient's journey to parenthood is inherently distinct.
The initial appointment focuses on determining the most suitable treatment plan. As a fertility doctor and the parent of four children conceived through IVF, Dr. Frattarelli understands the complexities and extensive information patients need. His team prioritizes educating the patients thoroughly, providing information through discussions, videos, and written materials via text or email to ensure they are well-informed about their options and treatment processes.
Patients face several challenges on their fertility journey, including accessing advanced treatments and managing emotional issues, making it essential to find a facility that meets their needs. FIH meets these challenges head-on by offering patients personalized care and emotional support throughout their fertility and early pregnancy journey. Expanding fertility treatments beyond infertile couples to single and transgender parents introduces complex legal and ethical considerations. FIH successfully guides its patients with a comprehensive understanding of FDA regulations and varying legal and ethical standards across countries and states.
One notable success story highlights FIH’s commitment to personalized care. A 44-year-old woman, who had undergone several unsuccessful treatments at another facility, came to FIH determined to fulfill her dream of becoming a mother. Typically, donor eggs are recommended for women at her age and with her reproductive history, but she was determined to use her eggs. Through a tailored treatment protocol of an IVF cycle, the expert team retrieved three eggs, one of which was successfully fertilized and developed into a blastocyst embryo. Due to the high risk of chromosomal abnormalities in embryos from older women leading to a miscarriage or a baby with Down syndrome or some other chromosomal defect, she opted for pre-implantation genetic testing, which confirmed the embryo was chromosomally normal. After preparing her uterus, FIH successfully transferred the embryo, resulting in a healthy pregnancy and baby birth, fulfilling her dream of becoming a mother.
A brand-new facility awaits FIH’s patients, which will be fully functional in July. This new space is nearly triple its current size, providing more room for its patients and updating its already state-of-the-art equipment, ensuring the best possible care.
Through personalized care, advanced treatments, and emotional support, the Fertility Institute of Hawaii continues to fulfill its patient’s dreams of parenthood by transforming hopes into reality—one baby at a time.
